Write a proper plugin. It sounds like a 'local' plugin to me. The docs are here - http://docs.moodle.org/dev/Local_plugins - it sounds a bit daunting, but you actually won't need much.
A version file, your page (index.php) and the language file with the 'pluginname' string defined.
