I just saw this bit of the thread:

... so I thought I would clarify. It sounds like youve though this through, If you cant add media to your questions, its probably just because the buttons arent being displayed by tinymce (fiddle about with text editor plugin settings in site admin).
If you are hellbent on adding a new question type, you could duplicate an existing one in moodle/question/type and have a good old fiddle(what do you want from the questions?)
